Medical Insurance: CORBI offers a preferred provider organization (PPO) network. Associates are eligible the first of the month following 45 days of employment. Deductibles are $250 for individual and $750 for family. Co-pays apply to certain services. Prescriptions for a 30 day supply are $10 / $25 / $40. CORBI pays for 77% of the premiums.

Dental Insurance: CORBI offers dental coverage through Delta Dental. Associates are eligible the first of the month following 45 days of employment. Associates are free to see any dentist; however the in-network fees are reduced along with out of pocket expenses. CORBI pays for 75% of the premiums.

Vision Insurance: CORBI offers dental coverage through Delta Dental. Associates are eligible the first of the month following 45 days of employment. A national network of providers gives associates a choice. CORBI pays for 75% of the premiums.

Short Term Disability: CORBI offers short term disability to all associates at no cost to the associate. Associates are eligible the first of the month following 45 days of employment. Disability insurance pays 60% of your earnings, up to $1000 per week, for a maximum of 26 weeks.

Long Term Disabiity: CORBI offers long term disability to all associates at no cost to the associate. Associates are eligible the first of the month following 45 days of employment. Disability insurance pays 60% of your earnings, up to $6,000 per month. Long term disability becomes effective 180 days following disability.

Life Insurance: CORBI offers life and accidental death & dismemberment insurance to all associates, at no cost to the associate, in the amount of two times your annual compensation. CORBI also offers dependent life insurance, at no cost to the associate, in the amount of $10,000 for spouse and $5,000 for child. Associates are eligible the first of the month following 45 days of employment.

Flexible Spending Account: CORBI offers a flexible spending account to set aside pre-tax dollars to pay for out of pocket medical and dental expenses, as well as dependent care expenses. Annual election locks in the payroll deduction for the year.

PTO: Associates accrue paid time off (PTO) based upon their anniversary date. New associates accrue a pro-rated 80 hours during their first calendar year. PTO is a combination of sick, vacation and personal time.

Holidays: CORBI normally observes 10 holidays each year. Dates vary based on the location and production needs.

401(k): All associates who have completed 9 months of service and attained age 21 are eligible to participate at the beginning of the next calendar quarter. Associates can contribute up to the IRS limits. CORBI currently matches 100% of the first 1% and 50% of the next 5%. Matching contributions are 100% vested after 2 years of employment.
